Student Care Concerns- Academic and Non-Academic Forms

Please submit the appropriate form depending on the type of concern. If the student is demonstrating both types of concerns, please submit one of each form. The appropriate group will follow up with the student. These forms are for traditional, undergraduate students.

** Non-Academic Concerns (Fill out the Student Care Form)

  • Abrupt change(s) in patterns of behavior
  • Substantial behavioral disruptions to the academic/campus community
  • Extended absence from campus
  • Financial difficulties
  • Family or personal emergencies
  • Considering withdrawal

The Student Care form is procesed by a Student Care team under the direction of Tyler Webb.


**Academic Concerns (Fill out the Early Academic Alert Form**).

  • Excessive Absences without communication or explanation
  • Non-completion of three or more graded assignments without communication
  • Need for academic remediation
  • Student is in danger of failing course (especially in the first 4 weeks of the semester)

The Early Academic Alert Form is processed by the Ally Center for coordination of support.


If you are concerned about a student who may be at risk of harm to themselves or others:

  • Dial 988 (9-988 from LBC phones). On July 16, 2022, the U.S. launched a new, easy-to-remember way for people to connect with help 24/7 during a mental health, substance abuse, or suicide crisis. People can call or text 988 or chat online (988lifeline.org) to be connected with a trained counselor. The 988 number will go directly to our Lancaster Crisis Intervention Team when dialled locally.
  • If you are unsure how to proceed related to a student's mental health on the Lancaster campus, and it is NOT an immediate, life-threatening emergency, call the Solutions Center (717-560-8254) during regular business hours 8am-5pm, and they will connect you with an on-call support staff member. After regular business hours, please call 988.
  • In case of an immediate life-threatening emergency, please dial 911 (9-911 from LBC phones).

Any level of concern regarding harm to self or others should always be handled immediately through real-time communication. A care form will NOT trigger an immediate emergency response. Care forms should be a secondary means to communicate details about emergencies after a student is safe.
